Registrations

New Patients:

If you have recently moved into the area and wish to register with us, please pop down to the surgery during our opening hours to collect a New Patient Registration Form. Alternatively, please e-mail NewQuaySurgery@Wales.nhs.uk to request an electronic version of the form. Please have all relevant information to hand, as we cannot register you without a fully completed form. 

The practice respects the right of patients to express a preference of practitioner when registering, however please be mindful that this cannot always be guaranteed when requesting an appointment. 

Temporary Patients:

Should you need medical assistance whilst visiting New Quay, you can register with us under a temporary basis. This will not effect your registration status with your current practice, it will simply allow us to put you on the system for any treatment provided.
